In June 2025, Sharp HealthCare implemented a workforce reduction, laying off 315 employees—approximately 1.5% of its total staff—primarily in non-patient care departments such as administration and support services, in response to escalating labor costs, seismic compliance requirements, and declining reimbursements from payers.^[98]^[99] The organization also adjusted hours and roles for additional positions and reduced executive compensation to align with these fiscal pressures, aiming to maintain clinical care quality without impacting patient-facing roles.^[100] Sharp HealthCare's strategic initiatives for 2025 emphasize leveraging artificial intelligence and digital tools to enhance efficiency and drive growth, as outlined by CEO Chris Howard in a February report. The system is optimizing its Epic electronic health record platform through a dedicated governance council, incorporating AI for clinical decision support and operational streamlining, while expanding ambulatory care via freestanding surgery centers to shift high-cost procedures and reduce inpatient burdens. These efforts build on revenue growth from a $3.5 billion baseline in 2016 to approximately $4.5 billion in fiscal 2022, with continued growth and projections targeting new income streams to offset over $1 billion in upcoming seismic upgrades and a state-mandated $25-per-hour minimum wage.^[76]^[16]^[101] Operationally, Sharp is addressing capacity constraints with hospital bed expansions amid over 90% average occupancy rates across its facilities, including seismic compliance upgrades for 175 beds at the affiliated Tri-City Medical Center through a new partnership approved in May 2025 and finalized in November 2025, under which Sharp will assume operations for 30 years.^[76]^[102]^[103] The Sharp Health Plan is being fortified as a key insurer, with 2025 enhancements to its Performance and Premier networks offering nine individual and family plan options that provide varying levels of coverage, deductibles, and provider access to improve affordability and care coordination in San Diego.^[104]

Controversies and challenges

In early 2025, Sharp HealthCare faced a significant data breach stemming from a ransomware attack on its business associate, Episource, a healthcare services firm. The incident occurred between January 27 and February 6, 2025, when cybercriminals gained unauthorized access to Episource's systems, compromising sensitive information including contact details, insurance data, and health records for affected individuals.^[105]^[106] The breach impacted over 5.4 million people across Episource's clients, with Sharp HealthCare notifying approximately 25,000 affected members of potential exposure.^[107]^[108] In response, Episource conducted an internal investigation, contained the breach, and enhanced its cybersecurity measures, while Sharp HealthCare issued notifications to impacted parties and advised them to monitor for identity theft and medical fraud.^[109]^[110] No evidence of data misuse was reported at the time of notifications filed with state authorities in June 2025.^[111] Labor disputes intensified in mid-2025 following Sharp HealthCare's announcement of layoffs affecting 315 employees, representing about 1.5% of its workforce, primarily due to rising operational costs.^[100]^[112] Former employees initiated a WARN Act investigation in July 2025, alleging the organization failed to provide the required 60-day advance notice for the mass layoffs, potentially entitling affected workers to back pay and benefits.^[113]^[114] In the aftermath, approximately 40 medical office workers across six Sharp facilities voted unanimously in July 2025 to unionize with SEIU-United Healthcare Workers West, citing concerns over job security, patient care quality, and working conditions exacerbated by the layoffs.^[115]^[116] Tensions escalated further in October 2025 when thousands of registered nurses and health professionals, represented by the Union of Nurses and Allied Care Professionals (UNAC/UHCP), held three days of informational pickets at Sharp facilities, protesting punitive sick leave policies that required full-time nurses to work at least 10 weeks to offset one sick shift, alongside demands for better staffing to mitigate patient safety risks.^[117]^[118]^[119] In November 2025, over 5,700 nurses voted 97% in favor to authorize a strike amid ongoing contract negotiations since the previous agreement expired on September 30, 2025, focusing on improved pay, sick leave accrual, and staffing levels.^[120] Sharp HealthCare also encountered legal challenges in other areas during 2025. In March 2025, the organization threatened litigation against Palomar Health, demanding repayment of $26 million in principal and interest from a prior loan, claiming Palomar breached an exclusivity agreement by securing alternative financing from UC San Diego Health.^[121]^[122]^[123] Earlier, in 2019, Sharp HealthCare settled whistleblower allegations under the Civil Monetary Penalties Law for $153,000, resolving claims that it employed an individual excluded from federal healthcare programs and engaged in improper billing practices.^[124] In May 2025, a California court granted de-certification of a class action lawsuit against Sharp HealthCare, which had alleged unfair billing practices under California's Unfair Competition Law and Consumers Legal Remedies Act over charges to uninsured patients during a multi-year period, determining that individual issues predominated over common ones.^[125]
